A partial assessment is the limited service that constitutes a history of the presenting complaint, the necessary physical examination, advice to the patient and appropriate record.
When to Use
- Bill A644 for a brief follow-up visit focused on a specific post-operative symptom, where a full reassessment (C644) or consultation (A645) is not warranted.
- Use A644 when a patient requires a limited assessment of a new thoracic complaint, but the physician does not need to perform a comprehensive history and physical examination as would be required for A645.
- Claim A644 as the adjusted fee when a more comprehensive assessment (e.g., A645) has exceeded its usage limits for the patient within the applicable timeframe.
Common Pitfalls
- Billing A644 for a hospital admission assessment when it is not the major pre-operative visit, as this is a common reason for claim rejection.
- Submitting A644 on the same day as an Independent Operative Procedure (IOP) without meeting the specific criteria related to a prior major pre-operative visit, leading to denial.
- Attempting to bill A644 for services that meet the criteria for a full consultation (A645) or specific reassessment (C644), resulting in incorrect payment or audit issues.
Billing Tips
- For virtual care, always append the 'A' suffix to A644 (i.e., A644A) to indicate video or telephone assessment.
