Concurrent care is any routine assessment rendered in a hospital by a consultant following the consultant's first major assessment of the patient when the family physician remains the most responsible physician but the latter requests continued directive care by the consultant. As an assessment, this service includes the specific elements of assessments as defined in , such as a direct physical encounter, history taking, physical examination, and providing advice to the patient.

When to Use

  • Use C118 when you have already performed an initial consultation (A-code) and the Most Responsible Physician (MRP) requests you to continue providing active, directive management for a specific condition.
  • Use C118 for ongoing daily or periodic hospital visits where you are actively managing a patient's care plan, as opposed to C002 (Supportive Care) which is for monitoring without active intervention.

Common Pitfalls

  • Billing C118 when the patient is no longer under the care of an MRP who has requested your directive input, which effectively turns the service into non-billable or supportive care.
  • Claiming C118 on the same day as your initial consultation (A-code); C118 is strictly for visits occurring after the initial major assessment is completed.
  • Failing to document the specific request from the MRP for continued directive care, which is a mandatory requirement for audit compliance.

Billing Tips

  • Ensure your documentation explicitly states the clinical reason for your continued involvement and the specific directive advice provided to the MRP to justify the 'directive care' requirement.
  • If the patient is in an ICU or CCU, remember to append the C101 premium to your C118 claim to maximize the value of the visit.
