C777Intermediate assessment - Pronouncement of death

OHIP Surgical Procedures Code — CONSULTATIONS AND VISITS · Schedule of Benefits

An intermediate assessment for the pronouncement of death for a non-emergency hospital in-patient, subject to the same conditions as A777. As per , this service is for pronouncing a patient dead in a location other than in the patient's home (in this case, a hospital). The service may include any counselling of relatives that is rendered during the same visit, and completion of the death certificate. The claim must be submitted using the diagnostic code for the underlying cause of death, as recorded on the death certificate, rather than the immediate cause of death.

When to Use

  • Use C777 when pronouncing a patient dead in a hospital setting where you are the attending physician or the physician on call for the patient.
  • Use this code when the pronouncement occurs during a routine visit or a non-emergency call to the hospital, distinguishing it from emergency-based codes like W777.

Common Pitfalls

  • Submitting the immediate cause of death (e.g., cardiac arrest) instead of the underlying cause of death as required by GP27, which leads to claim rejections.
  • Billing C777 in conjunction with other hospital visits on the same day, which often triggers a 'duplicate service' or 'already paid' rejection if not properly managed with a manual review or appropriate diagnostic coding.

Billing Tips

  • Ensure the diagnostic code used on the claim matches the underlying cause of death documented on the medical certificate of death to satisfy audit requirements.
  • If the pronouncement requires an urgent or emergency response, bill the appropriate emergency visit code instead of C777 to ensure the service is valued correctly for the time and urgency involved.
