This is an add-on fee payable for each additional metatarsal head excised, performed in the same operative session as R309A - Excision of metatarsal head. This code should be claimed for the second, third, etc., metatarsal head excision.
When to Use
- Use E587 for the second, third, or fourth metatarsal head excision when performing a multi-metatarsal resection alongside the primary R309A procedure.
- Apply E587 when addressing multiple symptomatic metatarsalgia sites in a single operative session, provided the primary R309A is billed for the first metatarsal head.
Common Pitfalls
- Billing E587 as a standalone procedure without an accompanying R309A will result in an automatic rejection.
- Claiming E587 for the first metatarsal head excision is incorrect; the first must always be billed under R309A.
- Over-billing by claiming E587 for the same metatarsal head already covered under the primary R309A fee.

- Ensure the quantity field for E587 accurately reflects the number of additional metatarsal heads excised beyond the first one covered by R309A.
