E594 – Each additional hammer toe

This fee is applicable for the surgical correction of each additional hammer toe when more than one is addressed in the same operative session. It is an add-on code and is only payable when billed with `R430` (Claw and hammer toe). As an 'each additional' code, it is not payable for the first hammer toe corrected.
When to Use
- Use E594 for the second, third, or subsequent hammer toe corrections performed during the same operative session as the primary R430 procedure.
- Apply this code when addressing multiple hammer toes on the same foot or across both feet if the primary R430 procedure was already claimed for the first digit.
Common Pitfalls
- Billing E594 as a stand-alone code without an associated R430 claim will result in an automatic rejection.
- Claiming E594 for the first hammer toe corrected is a common error; the first digit must always be billed under the primary R430 code.
- Failure to clearly document the number of additional toes corrected in the operative report can lead to claim adjustments during an audit.
