This service is for each additional pacemaker lead extraction and is billed in addition to Z428 (Pacemaker lead extraction, including the use of extraction sheathes, with or without laser or similar technology).
When to Use
- Use E628 for the second and each subsequent lead extracted during the same operative session as the primary lead extraction billed under Z428.
- Apply this code when multiple leads (e.g., atrial and ventricular) are removed, ensuring Z428 is billed for the first lead and E628 is billed for each additional lead.
Common Pitfalls
- Billing E628 without a corresponding Z428 on the same claim, which will result in an automatic rejection as it is an add-on code.
- Attempting to bill E628 for the first lead extraction, which must be captured under the primary procedure code Z428.
- Failing to specify the number of units for E628 when extracting three or more leads, as each additional lead beyond the first requires a separate unit count.
Billing Tips
- Ensure the total number of units for E628 reflects the total number of leads removed minus one, corresponding to the primary lead billed under Z428.
