A re-operation involving open heart procedures for a patient with a history of a previous thoracotomy. This service fee is applicable when the procedure is performed more than one month after the initial operation. This service includes all common and specific elements of a surgical procedure as defined in the Schedule of Benefits. Assistant and anaesthetist services are also applicable.
When to Use
- Use E670 for elective re-operations on the heart or great vessels when the patient has a documented prior thoracotomy performed more than 30 days prior.
- Select E670 over standard procedural codes when the surgical approach is significantly complicated by adhesions or anatomical distortion resulting from the previous thoracotomy.
Common Pitfalls
- Billing E670 for procedures performed within 30 days of the initial thoracotomy will result in an automatic rejection; ensure the date interval exceeds one full month.
- Failing to provide clinical justification for the complexity of the re-entry can lead to audit scrutiny, as E670 is intended to compensate for the increased surgical difficulty of repeat sternotomy or thoracotomy.
