E686 – Percutaneous abdominal abscess drainage - additional quadrant

This is an add-on service for percutaneous abdominal abscess drainage performed within each other abdominal quadrant, or the pelvis (if the initial abscess was not in the pelvis). This code is intended to be added to Z594.
When to Use
- Use E686 when you perform drainage on a second, distinct abscess located in a different abdominal quadrant than the primary abscess drained under Z594.
- Apply E686 for a pelvic abscess drainage if the primary procedure (Z594) was performed on an abscess located in a different abdominal quadrant.
Common Pitfalls
- Billing E686 without a corresponding Z594 on the same claim will result in an automatic rejection.
- Attempting to bill E686 for multiple drainages within the same quadrant is prohibited, as the code is strictly per additional quadrant, not per abscess.
- Failing to clearly document the anatomical location of each abscess in the operative report creates a high risk of recovery during an audit.
Billing Tips
- Ensure the operative note explicitly identifies the quadrant for the primary drainage (Z594) and the distinct quadrant for each E686 unit claimed to justify the add-on.
