When to Use
- Use E754 when you perform a pyeloplasty (S422) and must simultaneously remove a renal pelvic calculus identified during the reconstruction.
- Apply this code when the calculus removal is an incidental finding or secondary necessity during a planned pyeloplasty, regardless of whether the approach is open or laparoscopic (E792).

- Billing E754 as a standalone procedure will trigger an automatic rejection because it is strictly an add-on code requiring S422.
- Attempting to bill E754 alongside other stone removal codes like those for ureteroscopy (e.g., S425) for the same stone will be flagged as duplicate billing for the same clinical task.
