E765Liver transplant - reconstruction or repair of the hepatic artery

OHIP Surgical Assists Code — DIGESTIVE SYSTEM SURGICAL PROCEDURES · Schedule of Benefits

Add-on fee for liver transplant procedures that includes reconstruction or repair of the hepatic artery, such as re-anastomosis or using a conduit. This service is claimed in addition to the main liver transplant fee. See `S294, S295, S266`. This is an add-on to the liver transplant fee and is not a stand-alone procedure.

When to Use

  • Use E765 when performing a complex hepatic artery re-anastomosis necessitated by anatomical variation or vessel damage encountered during a liver transplant (S294, S295, or S266).
  • Apply this code when utilizing an interposition graft or conduit to bridge a gap in the hepatic artery during the transplant procedure.

Common Pitfalls

  • Billing E765 as a stand-alone procedure will result in an automatic rejection; it must be submitted on the same claim as the primary transplant code.
  • Attempting to claim E765 for routine arterial anastomosis inherent to the standard transplant procedure (S294/S295) will be flagged as unbundling.
  • Failing to provide specific operative notes detailing the reconstruction technique can lead to recovery during post-payment audits.

Billing Tips

  • Ensure the primary transplant code (e.g., S294) is listed first on the claim to establish the surgical context for the E765 add-on.
  • If the transplant occurs during after-hours, apply the E409 or E410 premium to the E765 fee to maximize the procedural reimbursement.
