F052 – Neck with dislocation of head - open reduction
OHIP Anaesthesia Code — MUSCULOSKELETAL SYSTEM SURGICAL PROCEDURES · Schedule of Benefits
This is a surgical procedure for the open reduction of a fracture of the neck of the humerus with dislocation of the head. This service involves surgically exposing the fracture site to realign the bone fragments and relocate the humeral head into the glenoid socket. This service is eligible for surgeon (F052A), assistant (F052B), and anaesthetist (F052C) fees. Payment for assistant and anaesthetist services is calculated based on a combination of basic units and time units.
When to Use
- Use F052 for open reduction and internal fixation (ORIF) of a proximal humerus fracture specifically complicated by a glenohumeral dislocation.
- Select F052 when the surgical approach requires formal arthrotomy or open exposure to achieve reduction, distinguishing it from closed manipulation under anesthesia (F050 or F051).
Common Pitfalls
- Billing F052 alongside closed reduction codes (F050/F051) is a common error; open reduction is inclusive of the reduction maneuver and cannot be unbundled.
- Failure to document the Injury Severity Score (ISS) in the medical record will result in the rejection of the E420 trauma premium, even if the procedure is clearly trauma-related.
- Attempting to bill for a second surgical assistant without prior written justification will lead to automatic denial, as F052 is not on the pre-approved list for dual assistance.
Billing Tips
- Ensure the operative report explicitly details the dislocation and the necessity of the open approach to justify the higher complexity of F052 over standard fracture fixation codes.
- When billing after-hours premiums (E409/E410), ensure the start time of the procedure is accurately reflected in the claim to avoid audit discrepancies regarding the 50% or 75% uplift.
