G123 – Paravertebral nerve block - additional nerve

This service is for each additional paravertebral nerve block of the cervical, thoracic, lumbar, sacral, or coccygeal nerves. It is intended to be billed in addition to G228 (Paravertebral nerve block of cervical, thoracic or lumbar or sacral or coccygeal nerves). A maximum of 4 additional blocks may be claimed per encounter.
When to Use
- Use G123 for each subsequent paravertebral nerve block performed after the initial block billed under G228 during the same session.
- Apply G123 when treating multiple levels or bilateral sites, provided the total count of additional blocks does not exceed four units.
Common Pitfalls
- Billing G123 without an accompanying G228 on the same claim will result in an automatic rejection.
- Claiming more than four units of G123 per encounter will trigger a rejection, as the Schedule of Benefits imposes a strict maximum of four additional blocks.
- Attempting to bill G123 for blocks performed on different days or separate encounters is invalid; it must be linked to the primary G228 procedure.
Billing Tips
- Ensure your claim includes one unit of G228 and the appropriate number of G123 units (up to four) to accurately reflect the total number of nerves blocked.
