Represents the physician's professional service for the reprogramming of a dual chamber device. This service includes the performance and interpretation of an associated electrocardiogram. It is intended to be billed for the physician's interpretive and cognitive work, and is billed separately from its corresponding technical component, G181.
In accordance with , all insured services must be documented in appropriate records to establish that: 1. an insured service was provided; 2. the service for which the account is submitted is the service that was rendered; and 3. the service was medically necessary.
