G258 – Intrapleural block (single injection)

An intrapleural block is a regional anaesthesia technique performed via a single injection into the intrapleural space. It is used for providing analgesia. See G257 for intrapleural block with catheter insertion for continuous analgesia.
When to Use
- Use G258 for a single-shot intrapleural injection intended for acute analgesia, such as pain management for rib fractures or post-thoracotomy pain.
- Select G258 when the procedure is performed as a standalone therapeutic intervention rather than as an anesthetic adjunct to a surgical procedure you are also performing.
Common Pitfalls
- Billing G258 alongside a surgical procedure you performed on the same patient will result in a rejection, as the block is considered inclusive of the surgical fee.
- Confusing G258 with G257; G258 is strictly for a single injection, whereas G257 must be used if a catheter is inserted for continuous infusion.
Billing Tips
- If you perform this procedure in an emergency or hospital setting after hours, ensure you append the appropriate Special Visit Premium to maximize the claim value.
- If the block is performed specifically for post-operative pain control by the surgeon, ensure you review the G224 exception criteria to determine if that code is more appropriate than G258.
