G275 – Exchange transfusion

An exchange transfusion is a procedure where a patient's blood is removed and replaced with donated blood or plasma, typically to remove toxins or abnormal blood components. This service is listed as a major invasive procedure and is therefore eligible for after-hours procedure premiums (E409, E410). Note: An assistant at an exchange transfusion may be payable as per the rules outlined in the General Preamble ().
When to Use
- Use G275 for neonatal exchange transfusions performed to manage severe hyperbilirubinemia or hemolytic disease of the newborn.
- Use G275 for therapeutic exchange transfusions in pediatric or adult patients requiring rapid removal of circulating toxins or abnormal blood components.
Common Pitfalls
- Attempting to bill an assistant's service using G275B, which is invalid; you must use M400B for assistant claims.
- Failing to submit the required letter from the primary surgeon justifying the need for an assistant, which will result in the rejection of the M400B claim.
- Billing G275 as an elective procedure; the after-hours premiums E409 and E410 are only payable for non-elective procedures or those delayed by surgical emergencies.
Billing Tips
- Always append the appropriate after-hours premium (E409 or E410) to G275 if the procedure meets the non-elective criteria and falls within the specified time windows.
- When billing for an assistant, submit M400B with the supporting documentation to the medical consultant for manual authorization as per GP85.
