Telephone supervisory fee for ovulation induction with human menopausal gonadotropins or gonadotropin-releasing hormone (not eligible for payment same day as visit), to a maximum of 10 per cycle per call.
When to Use
- Use G334 for telephone calls to adjust dosages of human menopausal gonadotropins or GnRH during an ovulation induction cycle.
- Apply this code when providing remote clinical oversight for patients undergoing non-IVF ovulation induction protocols.
Common Pitfalls
- Billing G334 on the same day as any assessment code (e.g., A007, A001) will result in an automatic rejection.
- Submitting G334 for patients undergoing IVF or artificial insemination is a violation of Regulation 552 and will lead to clawbacks during an audit.
- Exceeding the maximum of 10 claims per cycle will trigger a rejection for the 11th and subsequent calls.
