G353 – Esophageal provocation testing

Esophageal provocation testing is only eligible for payment when one or more of the following tests are performed: 1) acid perfusion testing; 2) provocative drug testing; 3) multiple rapid swallows; 4) rapid drink challenge; 5) solid bolus swallows.
When to Use
- Use G353 when performing an acid perfusion (Bernstein) test to evaluate for esophageal sensitivity in patients with non-cardiac chest pain.
- Use G353 when conducting a rapid drink challenge or multiple rapid swallows during high-resolution manometry to assess esophageal inhibition or peristaltic reserve.
- Use G353 when administering provocative drug testing, such as edrophonium, to elicit esophageal spasm symptoms during manometric evaluation.
Common Pitfalls
- Billing G353 in conjunction with G350 (Esophageal manometry) without documenting the specific provocation maneuver performed, as G353 is intended as an add-on or distinct diagnostic procedure.
- Submitting G353 for routine esophageal manometry or standard pH monitoring when no specific provocation test listed in the schedule was actually performed.
- Failing to include the required referral information from a physician or nurse practitioner, which is mandatory for this diagnostic procedure.
Billing Tips
- Ensure the specific provocation method (e.g., solid bolus swallows) is clearly noted in the procedure report to justify the separate claim for G353 alongside other diagnostic codes.
- Review GP15 rules to ensure that if an assessment is performed during the same encounter, it is billed separately only if it meets the criteria for a distinct service from the G353 procedure.
