G398 – Pessary fitting

Medical management of prolapse using a pessary. This service covers the initial fitting or a subsequent re-fitting of the device. According to the commentary on page , a visit fee may be claimed in addition to pessary fitting or pessary care provided to the same patient during the same visit.
When to Use
- Use G398 when performing the initial sizing and insertion of a vaginal pessary for pelvic organ prolapse management.
- Use G398 for a subsequent visit where the patient requires a re-fitting or replacement of the device due to size changes or device degradation.
Common Pitfalls
- Billing G398 on the same day as G550 (pessary removal/cleaning) will result in a rejection, as these are mutually exclusive.
- Claiming G398 for a simple pessary removal without re-insertion or re-fitting is an audit risk, as the code specifically requires the management/fitting procedure.
Billing Tips
- Always append a visit fee (e.g., A007) to your claim for G398, as the Schedule of Benefits explicitly permits both to be billed for the same encounter.
