Hyperbaric Oxygen Therapy (HBOT) is the service rendered when a physician administers and supervises HBOT. Time is calculated based on the period of physician supervision while each patient receives HBOT inside the chamber. The specific elements of HBOT are those of an assessment, including ongoing monitoring of the patient's condition and intervening as appropriate. This service is for the physician in constant attendance inside the chamber with the patient(s), per session per patient, for each quarter hour or major part thereof after the initial two hours in the chamber. See also G800 and G801 for time preceding this service.

When to Use

  • Use G802 for each 15-minute increment of physician supervision after the initial 120 minutes of a hyperbaric session have been billed using G800 and G801.
  • Apply this code specifically when the physician remains in constant attendance inside the chamber with the patient beyond the two-hour threshold.

Common Pitfalls

  • Billing G802 for time spent outside the chamber when the physician is not in constant attendance, which violates the requirement for continuous supervision.
  • Failing to subtract time spent on separately billable interventions if the physician is outside the chamber, as G802 billing rules differ based on location.
  • Overlapping G802 with G807, which is explicitly restricted and will result in claim rejection.

Billing Tips

  • Ensure your medical record clearly denotes the exact start and stop times for the portion of the session exceeding two hours to justify the number of G802 units claimed.
  • You may include time spent on separately billable interventions within G802 units only if the physician is inside the chamber providing continuous, uninterrupted supervision.

The start and stop times of the service are recorded in each patient's permanent medical record.

HBOT is insured only for the treatment of those internationally recognized indications approved by the ministry.

HBOT is only eligible for payment for idiopathic sudden sensorineural hearing loss (ISSHL) when the following conditions are met: a. The patient is treated concurrently with corticosteroid unless corticosteroids are contraindicated; b. The treatment is initiated within 30 days of a diagnosis of ISSHL; and c. The diagnosis of ISSHL is made or confirmed by an Otolaryngologist prior to the initiation of treatment.

HBOT is only eligible for payment for avascular necrosis (AVN) of bone following diagnostic imaging confirmation of symptomatic early to mid-stage AVN, e.g. Ficat stage I, II, or III or equivalent.

As of April 1, 2026, the following indications were approved by the MOH:

• air or gas embolism

• carbon monoxide poisoning and/or cyanide poisoning

• clostridial myositis and myonecrosis (gas gangrene)

• crush injury, compartment syndrome, and other acute traumatic ischemias

• decompression sickness

• enhancement of healing in selected problem wounds

• exceptional blood loss

• intracranial abscess

• necrotizing soft tissue infections (subcutaneous tissue, muscle, fascia)

• osteomyelitis (refractory)

• delayed radiation injury (soft tissue and bony necrosis)

• skin grafts and flaps (compromised)

• thermal burns
