Selective catheterization is the manipulation of a catheter from the vessel of introduction into a branch tributary or cardiac chamber with angiogram(s). This fee is an add-on to the primary catheter insertion fee, such as J021, and is claimed for each vessel catheterized, up to a maximum of four vessels. The term 'Selective' is defined as the manipulation of the catheter from the vessel of introduction into a branch tributary, or cardiac chamber with angiogram(s), as described for spinal and parathyroid angiography (J014).
When to Use
- Use J022 when you have successfully manipulated a catheter from the primary insertion site into a specific branch tributary to perform an angiogram.
- Claim J022 for each distinct vessel catheterized beyond the initial vessel, up to a maximum of four separate vessels per procedure.
- Use this code specifically for selective procedures that include the performance of an angiogram, distinguishing it from non-selective diagnostic imaging.
Common Pitfalls
- Do not claim J022 for the primary vessel of introduction; it is strictly an add-on for secondary or tertiary selective catheterizations.
- Avoid billing J022 in conjunction with cardiac catheterization codes (Z439 to G288), as these are explicitly restricted by the Schedule of Benefits.
- Ensure you do not exceed the four-vessel maximum; any additional selective catheterizations beyond four are not eligible for separate fee-for-service payment.
Billing Tips
- Always pair J022 with the primary catheter insertion fee (J021) to ensure the claim is processed as a complete procedural package.
- If performing multiple selective catheterizations, ensure each vessel is clearly identified in the operative report to support the number of units claimed.
