When to Use

  • Use J028 for the performance of a retrograde urethrogram or cystogram to evaluate urethral trauma or bladder integrity.
  • Use J028 for imaging an intestinal conduit (ileal conduit) to assess for leaks or obstruction in a patient with a urinary diversion.
  • Use J028 when performing a combined urethrocystogram to visualize both the bladder and urethra simultaneously.

Common Pitfalls

  • Do not bill J028 in conjunction with Z540, as Z540 specifically refers to intubation of the small intestine and is not a substitute for urinary tract imaging.
  • Avoid billing J028 for simple diagnostic imaging interpretations; it is a procedural code intended for the performance of the examination, not the professional fee for reading the film.
  • Failure to include a valid referring physician or nurse practitioner on the claim will result in automatic rejection, as this code requires a formal referral.
