This service is for a transcranial doppler assessment of the intracranial circulation that is prolonged, requiring a minimum duration of 50 minutes. This service is for the professional component only (P fee). As per :, the start and end times of the service must be recorded in the patient's medical record. It is not payable on the same day as J186, J188, or J189.
When to Use
- Use J187 for complex transcranial doppler studies that require a minimum of 50 minutes of active assessment time, such as monitoring for vasospasm in subarachnoid hemorrhage patients.
- Select this code when the clinical complexity necessitates a prolonged examination duration, distinguishing it from the standard assessment covered by J186.
Common Pitfalls
- Failure to explicitly document the start and end times in the patient's chart will result in an automatic rejection or clawback during an audit.
- Attempting to bill J187 on the same day as J186, J188, or J189 will lead to a claim rejection, as only one of these codes is payable per patient, per physician, per day.
