A specific neurocognitive assessment is an assessment of neurocognitive function rendered personally by the physician where all of the following requirements are met: - test of memory, attention, language, visuospatial function and executive function. - a minimum of 20 minutes (consecutive or non-consecutive) and must be dedicated exclusively to this service (including administration of the tests and scoring) and must be completed on the same day; and - the start and stop time(s) must be recorded in the patient's medical record.
When to Use
- Use K032 when performing a comprehensive battery like the Behavioral Neurology Assessment (BNA) or Dementia Rating Scale (DRS) to evaluate cognitive impairment.
- Select K032 for patients requiring a formal, physician-administered assessment that exceeds the scope of a standard office visit (A007) or periodic health exam.
Common Pitfalls
- Billing K032 for a Mini-Mental State Examination (MMSE) will result in rejection or clawback, as it is explicitly excluded by the Schedule of Benefits.
- Failing to record exact start and stop times in the chart is a primary audit trigger that leads to automatic recovery of the $80.00 fee.
- Attempting to bill K032 and K042 on the same day for the same patient is prohibited and will cause a payment rejection.
Billing Tips
- Ensure the 20 minutes of time is documented as exclusively dedicated to the assessment, excluding time spent on unrelated chart review or administrative tasks.
- If the patient was recently discharged from an acute care hospital, remember to append the E080 premium to your K032 claim to capture the additional $25.25.
