When to Use
- Use K038 when completing the mandatory health assessment form required for a patient's formal application to a long-term care facility.
- Bill this code when you have finalized the clinical assessment and submitted the documentation to Ontario Health atHome, regardless of whether the patient is present in the office.

- Do not bill K038 for general administrative tasks or routine forms that are not part of the official Ontario Health atHome long-term care application process.
- Avoid billing K038 on the same day as a routine office visit (A007) if the assessment was performed during that visit, as the assessment fee is intended to cover the specific work of the application form.
