K077 – Geriatric telephone support
OHIP Consultation & Visit Premium Codes Code — CONSULTATIONS AND VISITS · Schedule of Benefits
This is the service initiated by a caregiver where a physician provides telephone support to a caregiver(s) for a patient with an established diagnosis of dementia.
When to Use
- Use K077 when a caregiver calls for guidance on managing behavioral symptoms or medication side effects for a patient with a confirmed dementia diagnosis.
- Use this code for discussions regarding the coordination of home care or safety planning for a dementia patient when the patient is not present on the call.
- Use K077 instead of K005 when the support is specifically directed at the caregiver of a dementia patient rather than the patient themselves.
Common Pitfalls
- Billing K077 for calls involving the patient directly; this code is strictly for caregiver-initiated support regarding a patient with dementia.
- Failing to document the exact start and stop times, which leads to automatic rejections during ministry audits.
- Attempting to bill K077 on the same day as a comprehensive assessment or another visit for the same patient, which may trigger a duplicate service flag.
Billing Tips
- Ensure the patient's dementia diagnosis is clearly noted in the medical record to justify the use of K077 over general telephone advice codes like K005.
- Record the name of the caregiver and their relationship to the patient in the chart to satisfy the specific documentation requirements for this service.
