K714 – Multidisciplinary epilepsy surgery case conference

Participation by the physician most responsible for the care of the patient and at least 2 other participants (physicians, regulated social workers and/or regulated health professionals) regarding a patient who may be a candidate for epilepsy surgery.
When to Use
- Use K714 when conducting a formal, pre-scheduled multidisciplinary review specifically to determine surgical candidacy for a patient with refractory epilepsy.
- Use this code when coordinating care with at least two other regulated health professionals, such as a neurologist, neurosurgeon, and neuropsychologist, to finalize a surgical plan.
Common Pitfalls
- Billing K714 on the same day as an office visit or any other insured service for the same patient will result in a rejection, as it is strictly prohibited.
- Attempting to bill K714 for general epilepsy management or routine follow-up discussions that do not specifically focus on surgical candidacy will lead to audit recovery.
- Failing to document the specific start and stop times or the names of the two required participants will result in a failed audit for this code.
Billing Tips
- Ensure the documentation explicitly states the patient is being evaluated for epilepsy surgery, as this is a mandatory condition for K714 that distinguishes it from general K700 series conferences.
- Since K714 allows up to 8 units per day, ensure your documentation clearly reflects the total duration of the discussion to justify multiple units if the conference exceeds 10 minutes.

Case Conference
Consultation
The conference must be pre-scheduled.
The purpose must be discussing and directing the management of an individual patient.
There must be a minimum of 10 minutes of patient-related discussion.
The physician must be actively participating, and this must be evident in the medical record.
The conference must be conducted by personal attendance, videoconference, or telephone (or a combination thereof).
Medical record must include: patient identification, start and stop times of the discussion, identification of participants, and the outcome/decision.
Regulated social worker refers to one registered under the Social Work and Social Service Work Act.
Case conferences for educational purposes (rounds, journal club) are not eligible.
Not eligible if participants necessary to meet the minimum requirements are paid by the claiming physician.
