- Billing L049 on the same day as an office visit code (e.g., A007) is considered unbundling, as the collection fee is intended to be inclusive of the visit unless specifically permitted by the Schedule of Benefits.
- Submitting L049 for specimens collected in a hospital or community laboratory setting will result in rejection, as this fee is strictly for office-based collection.
