Laboratory services are only eligible for payment if the result of the test(s), the physician’s interpretation of the results, and the treatment decision based on the results are documented in the patient’s permanent medical record.
If multiple specimens are submitted from a single patient on the same occasion, each specimen must be assigned the appropriate fee schedule code(s).
