When to Use
- Use L105 for the collection of a venous blood specimen by venipuncture when performed as a standalone service.
- Bill this code when a patient presents specifically for blood work ordered by another physician or for routine monitoring where no other assessment code is claimed.
Common Pitfalls
- Do not bill L105 in addition to an office visit assessment code (e.g., A007) as the venipuncture is considered bundled into the assessment fee.
- Avoid billing L105 if the blood draw is performed by a nurse or lab technician in a facility where the physician is not personally performing the procedure.

- Ensure the laboratory requisition clearly indicates the date and time of the draw to support the claim if audited for service delivery.
