When to Use
- Use L115 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, when the procedure is performed via simple excision.
- Select L115 instead of G005 or G006 when the lesion is benign and does not require the complex closure or tissue handling associated with malignant lesion excisions.
Common Pitfalls
- Billing L115 in conjunction with a minor assessment code (A007) is often rejected unless the assessment is for an unrelated clinical issue documented with a separate diagnosis code.
- Attempting to bill L115 for multiple lesions on the same day will trigger a reduction in payment for subsequent procedures; ensure you use the appropriate multiple procedure indicator (e.g., 50% for the second lesion).
Billing Tips
- If the lesion is removed via cryotherapy or curettage rather than excision, bill the appropriate minor procedure code instead of L115 to avoid audit scrutiny regarding the method of removal.
