When to Use
- Use L131 for the excision of a malignant lesion on the face, ears, nose, lips, eyelids, eyebrows, genitalia, or hands where the lesion diameter is 0.5 cm or less.
- Select L131 instead of L132 when the lesion size is 0.5 cm or less, as L132 is reserved for lesions greater than 0.5 cm up to 1.0 cm.
Common Pitfalls
- Billing L131 in addition to a minor assessment code (A007) on the same day is generally rejected unless the assessment is for a separate, unrelated clinical issue.
- Submitting L131 for a benign lesion excision is a common audit trigger; ensure the pathology report confirms malignancy to support the use of the L-series excision codes.
Billing Tips
- If the excision requires a complex closure, you may be eligible to bill an additional fee code for the repair, provided the documentation clearly describes the complexity of the reconstruction.
