When to Use
- Use L135 for the collection of a cervical or vaginal smear for cytology when the primary purpose of the visit is the screening or diagnostic investigation of the patient.
- Bill L135 when performing a Papanicolaou test as part of a routine health examination or follow-up for abnormal cytology.
Common Pitfalls
- L135 is a laboratory collection fee and cannot be billed in addition to an office visit fee (e.g., A007) if the smear is taken during the same encounter.
- Billing L135 for a self-collected sample or when the patient performs the collection is an audit risk, as the code requires the physician to perform the collection.
Billing Tips
- If you are performing a comprehensive physical examination, bill the appropriate visit code (e.g., K013) and omit L135, as the collection is considered bundled into the examination fee.
