When to Use
- Use L174 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, when performed via simple excision.
- Select L174 when the procedure involves the removal of a single lesion that does not require the complexity of a formal excision with layered closure, which would instead be billed under the G-code series for minor surgery.
Common Pitfalls
- Billing L174 in conjunction with an office visit fee (A007) is generally disallowed unless the excision is performed for a separate, unrelated clinical issue documented with a distinct diagnosis code.
- Submitting L174 multiple times for the same patient encounter is often flagged; use the appropriate multiple lesion modifiers or higher-value excision codes if removing multiple lesions in one session.
