- Billing L210 in conjunction with an office visit (A007) is generally disallowed unless the excision is a separate, identifiable service performed for a distinct clinical issue.
- Submitting L210 for multiple lesions on the same day often triggers rejections; ensure you use the appropriate multiple procedure rules or suffix codes if applicable to your specialty.
