When to Use

  • Use L211 for the simple excision of a single benign skin lesion, such as a skin tag or small seborrheic keratosis, where the procedure is performed under local anesthesia.
  • Select L211 when the lesion is removed via shave or snip excision that does not require complex closure or layered suturing.

Common Pitfalls

  • Billing L211 in addition to a minor assessment code (A007) for the same patient on the same day is often rejected unless a separate, unrelated clinical issue is documented.
  • Submitting L211 for lesions requiring excision with sutures, which should instead be billed under the Z-series codes (e.g., Z001) based on the size and anatomical location of the lesion.

Billing Tips

  • If multiple benign lesions are removed during the same encounter, bill L211 for the first lesion and use the appropriate multiple procedure rules for subsequent excisions to ensure correct payment.
