When to Use

  • Use L229 for the excision of a single benign lesion, such as a skin tag or small fibroma, that does not require a complex closure or pathology submission.
  • Select L229 when performing a simple excision of a lesion on the trunk or extremities where the procedure is straightforward and does not meet the criteria for the higher-valued Z-codes or S-codes.

Common Pitfalls

  • Billing L229 in addition to a minor assessment code (A007) for the same patient on the same day will result in a rejection for duplicate service.
  • Attempting to bill L229 for a lesion removal that requires sutures or complex excision will lead to audit scrutiny; use the appropriate Z-series excision codes for those procedures instead.

Billing Tips

  • Ensure the lesion site is clearly documented in the chart, as L229 is frequently flagged for manual review if the clinical necessity of the excision is not explicitly stated.
