When to Use
- Use L300 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, that does not require formal closure or complex reconstruction.
- Select L300 when performing a simple shave or snip excision of a lesion where the pathology is not required or is considered incidental to the primary visit.
Common Pitfalls
- Do not bill L300 in conjunction with a minor assessment code like A007, as the assessment is considered included in the procedure fee.
- Avoid billing L300 for multiple lesions on the same day without using the appropriate multiple procedure rules, as this often triggers an automatic rejection for duplicate service.
- Do not use L300 for lesion removals that require suturing or complex excision techniques, as these should be billed under the G-series surgical codes.
Billing Tips
- If removing multiple lesions, bill the first lesion at 100% and subsequent lesions at 50% using the appropriate suffix to ensure correct payment processing.
