  • Billing L301 in conjunction with a minor assessment code (A007) on the same day is often rejected unless the assessment is for an unrelated condition.
  • Avoid billing L301 for multiple lesions in the same anatomical area; use the appropriate multiple excision codes if the total size or count exceeds the single lesion definition.
