  • Select L305 instead of L300 when the lesion is benign and does not require the more extensive surgical intervention associated with the higher-valued excision codes.

Common Pitfalls

  • Billing L305 in conjunction with a minor procedure code like G538 is often rejected as the minor procedure is considered inclusive of the lesion removal.
  • Submitting L305 for multiple lesions on the same day without using the appropriate diagnostic codes or procedure modifiers will lead to automatic rejection for duplicate service.
