When to Use
- Use L313 for the excision of a single benign lesion, such as a skin tag or small fibroma, that does not require formal surgical closure or complex pathology.
- Select L313 when performing a simple shave or scissor excision of a superficial lesion that is not eligible for the higher-valued excision codes like S061 or S062.
Common Pitfalls
- Billing L313 in conjunction with a minor assessment code (A007) is often rejected; ensure the service is billed as a standalone procedure unless a separate, distinct clinical issue is addressed.
- Submitting L313 for a lesion requiring sutures or complex tissue handling will lead to an audit risk, as these procedures should be billed under the S-series excision codes.
Billing Tips
- If you perform multiple L313 excisions in the same encounter, apply the appropriate multiple procedure rule (usually 100% for the first and 85% for subsequent) to avoid manual adjustment by the Ministry.
