OHIP Dermatology Code · Schedule of Benefits

When to Use

  • Use L322 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, when the procedure is performed in a clinical setting.
  • Select L322 when the lesion excision does not require complex closure techniques or extensive tissue undermining that would otherwise necessitate a higher-valued procedure code.

Common Pitfalls

  • Billing L322 in conjunction with a minor assessment code (A007) is a common rejection; the assessment is generally considered included in the procedure fee.
  • Submitting L322 for multiple lesions on the same day without using the appropriate diagnostic code or multiple procedure indicators often leads to automatic claim rejection.

Billing Tips

  • If performing multiple excisions, ensure you apply the appropriate multiple procedure rules, as subsequent lesions billed under L322 are typically paid at 50% of the original fee.
