- Select L322 when the lesion excision does not require complex closure techniques or extensive tissue undermining that would otherwise necessitate a higher-valued procedure code.
Common Pitfalls
- Billing L322 in conjunction with a minor assessment code (A007) is a common rejection; the assessment is generally considered included in the procedure fee.
- Submitting L322 for multiple lesions on the same day without using the appropriate diagnostic code or multiple procedure indicators often leads to automatic claim rejection.
