When to Use
- Use L400 for the collection of a venous blood sample by venipuncture when performed by the physician or their staff.
- Bill L400 when a patient presents specifically for blood work that is not part of a comprehensive assessment or procedure where the fee is already inclusive.
Common Pitfalls
- Do not bill L400 in addition to an office visit fee like A007, as the collection fee is considered inclusive of the assessment.
- L400 is strictly for venipuncture; it cannot be billed for capillary blood draws or finger-prick glucose testing.
Billing Tips
- Ensure the laboratory requisition is clearly documented in the chart to support the medical necessity of the venipuncture if audited.
