When to Use
- Use L405 for the collection of a venous blood sample by venipuncture when the physician performs the draw personally.
- Bill L405 when a patient requires a blood draw for laboratory testing during an office visit where no other procedure code is applicable for the collection.
Common Pitfalls
- L405 is considered a 'by report' or minor service that is often bundled into the assessment fee (A007/A001) if performed during the same encounter.
- Billing L405 in addition to a minor surgical procedure code or a specific diagnostic test that already includes the cost of specimen collection will result in a rejection for duplicate billing.
Billing Tips
- Ensure the laboratory requisition clearly indicates the physician performed the venipuncture to support the claim if audited for the necessity of the service.
