When to Use
- Use L420 when performing a simple incision and drainage of a superficial abscess, boil, or furuncle that does not require extensive surgical debridement.
- Use L420 for the drainage of a paronychia or a localized infected hematoma when the procedure is limited to a simple stab incision.
Common Pitfalls
- Billing L420 in addition to a minor assessment code like A007 is often rejected unless the assessment is for an unrelated condition, as the assessment is considered included in the procedure.
- Attempting to bill L420 for complex abscesses requiring deep tissue debridement or packing will lead to underpayment; use S060 or appropriate surgical codes for those cases.
Billing Tips
- If the procedure requires a sterile tray, ensure you bill the appropriate tray fee (e.g., E409) alongside L420 to recover supply costs.
