When to Use

  • Use L440 when performing a diagnostic or therapeutic aspiration of a joint or bursa, such as a knee or shoulder, in an office setting.
  • Use L440 for the aspiration of a cyst or ganglion when the procedure is performed as a standalone service rather than part of a minor surgery.
  • Use L440 when performing a diagnostic aspiration of a hematoma or seroma that does not require the more complex incision and drainage procedures defined by codes like Z315.

Common Pitfalls

  • Billing L440 in conjunction with a minor surgery code (e.g., Z315) for the same site will result in a rejection for duplicate service or unbundling.
  • Attempting to bill L440 alongside a consultation or assessment code (A007/A005) is often flagged unless the procedure is medically necessary and distinct from the assessment.
  • Failing to include the specific anatomical site in the claim submission can lead to manual review or rejection by the Ministry.

Billing Tips

  • If you perform an injection (e.g., corticosteroid) immediately following the aspiration, you may bill the injection code (G372) in addition to L440, provided you use the appropriate diagnostic code for each.
