When to Use

  • Use L520 for the simple excision of a benign skin lesion, such as a skin tag or small seborrheic keratosis, where the procedure does not require complex closure or deep tissue dissection.
  • Select L520 when performing a minor surgical procedure that is not covered by more specific excision codes like Z001 or Z002, which are reserved for larger or more complex lesions.

Common Pitfalls

  • Billing L520 in addition to a minor assessment (A001) on the same day is often rejected unless the assessment is for a separate, unrelated clinical issue.
  • Submitting L520 for the removal of a malignant lesion is an audit risk; use the appropriate Z-code series for biopsies or excisions of suspected malignancies.

Billing Tips

  • If multiple lesions are removed during the same encounter, bill L520 for the first lesion and use the appropriate multiple procedure rules or secondary codes for subsequent removals to ensure proper payment.
