- Use L545 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, when the procedure is performed via simple excision.
- Select L545 when the lesion is removed by sharp dissection or curettage rather than by cryotherapy, which would be billed under a different modality.
Common Pitfalls
- Billing L545 in addition to a minor assessment code (A007) is often rejected unless the assessment is for a separate, unrelated clinical issue.
- Submitting L545 for multiple lesions on the same day is a common audit trigger; ensure you are using the appropriate multiple procedure rules if applicable.
