L575
L575 – L575
OHIP Dermatology Code · Schedule of Benefits
When to Use
- Use L575 for the excision of a single benign lesion, such as a skin tag or small seborrheic keratosis, that does not require formal pathology.
- Select L575 when performing a simple shave or scissor excision where the procedure is too minor to justify the higher complexity and fee of a Z-code excision (e.g., Z301).
Common Pitfalls
- Billing L575 in conjunction with a consultation or assessment code on the same day is often rejected unless the lesion removal is medically necessary and distinct from the assessment.
- Attempting to bill L575 for multiple lesions on the same site will result in a rejection; use the appropriate multiple lesion modifier or code for each distinct site if applicable.
Billing Tips
- Ensure the clinical note clearly distinguishes the procedure from a simple diagnostic assessment to avoid the 'included in the visit' rejection.
