When to Use

  • Use L582 for the excision of a malignant lesion on the face, scalp, neck, genitalia, hands, or feet where the lesion size is between 0.5 cm and 1.9 cm.
  • Select L582 instead of L581 when the lesion diameter exceeds 0.5 cm, as L581 is restricted to lesions smaller than 0.5 cm.

Common Pitfalls

  • Billing L582 alongside a consultation or assessment code (A007/A005) is often rejected unless the assessment is for a separate, unrelated clinical issue.
  • Submitting L582 for a benign lesion is a common audit trigger; ensure the pathology report confirms malignancy to support the use of the L5xx series.

  • Always document the exact diameter of the lesion in the chart, as the Ministry may request this to verify the L582 fee code selection against smaller or larger excision codes.
