When to Use
- Use L603 for the simple excision of a nail bed or nail matrix, typically performed for chronic ingrown toenails or localized nail bed pathology.
- Select L603 when the procedure is limited to a partial nail avulsion or matrixectomy that does not require the more extensive tissue reconstruction associated with Z305.
Common Pitfalls
- Billing L603 in conjunction with a minor assessment code (A007) is frequently rejected unless a separate, unrelated clinical issue is addressed and documented.
- Submitting L603 for a simple nail avulsion without matrix destruction may be flagged if the clinical notes do not support the surgical nature of the procedure.
Billing Tips
- If performing a bilateral procedure, append the appropriate bilateral modifier to L603 to ensure the second nail is paid at 50% or 100% depending on current Schedule of Benefits rules.
